Dr. Leila Kasrai – Placement of Breast Implants

Depending on the result you're looking for, as well as your body type, Toronto cosmetic surgeon Dr. Kasrai can place the implant in one of two positions: under the pectorals (chest muscle), or over it. She will determine the best approach for you during your consultation.

Under the muscle (subpectoral)


This approach positions the implant between the pectoral muscle and the rib cage.
Generally, thin patients with small, firm breasts are the best candidates for under-muscle placement. The advantages are:

  • A soft, natural-shaped breast
  • Less likely to cause rippling of the breast
  • A decreased likelihood of contracture (hardening)
  • Less likely to be able to see the edge of the implant (which can happen on women who are very thin)

The main disadvantage is that if you move your pectoral muscles, the breast may move as well.

Over the muscle (subglandular)

In this technique, Dr. Kasrai positions the implants above the muscle, and just behind the breast tissue. The best candidates for this approach are women with larger breasts and increased sagginess, with very little volume in the upper breast. It's a short, relatively easy procedure with less post-surgery discomfort.
